Please Read Before Submitting Any Forms
🚨 IMPORTANT: If you are starting from scratch and 
submitting a brand-new event, you must begin with the 
Submit an Event for Approval form only!

🚫 Do not fill out any other form: you only need to complete the Submit an Event for Approval form to begin and complete the entire process.

The additional forms are support forms used throughout the event and announcement process during our ongoing implementation and transition at New Home Ministries.

Each form below serves a specific purpose and has different requirements. Submitting the wrong form can delay your event planning process. Please read the description under each form to ensure you're submitting the correct one for your current stage.

Submit an Announcements for Approval

Use this form to request that a message or announcement be shared with the congregation. This helps keep communication clear and coordinated across all platforms.

Do NOT fill out this form if:
  • You're trying to get an event approved. Instead, start with the Submit an Event for Approval Form.

Submit an Event for Approval

Use this form to request approval for any upcoming church event. It ensures leadership is aware and can officially confirm the event aligns with ministry scheduling and priorities.

  • Please do not submit any additional forms if you are requesting a brand-new event.

Who should fill out this form:
  • Anyone who is starting from scratch and needs to submit a new event for approval.
  • If your event has not yet been confirmed by leadership or admin, this is your first and only step to begin the event planning process.
  • This form is required so leadership can evaluate the event’s alignment with the ministry calendar and needs.

After your event is reviewed and approved, you will receive a follow-up email directly from admin with a link to the Event Creation Request Form. This full planning form includes graphics, platform scheduling, and everything needed to move your event forward.


🔒 You will not have access to the Event Creation Request Form unless your event is approved  it is not publicly available and will only be sent via email by the admin team.


This is the only form you need to submit if you're requesting a new event from the beginning.

Do NOT fill out this form if:
  • Do not complete this form if your event has already been approved. 
  • Instead, use the Approved Scheduled Events Form to record the event on the calendar.

Again: Please do not submit any additional forms if you are requesting a brand-new event.

TRANSITION FORMS

Approved Scheduled Events 

Use this form to confirm and record events that have already been approved. If your event has not yet been approved, please fill out the Submit an Event for Approval Form.

Who should fill out this form:
  • Anyone whose event has already been officially approved and now needs to have it added to the ministry calendar and planning systems.
  • If you have a current flyer, you can upload it directly with this form.
  • If you do not have a flyer yet, but your event is approved, you can still submit this form but you must also complete the Graphic Design Request Form so the design team can create one for you.

Do NOT fill out this form if:
  • Your event has not yet been approved. Instead, complete the Submit an Event for Approval Form first.

Support Forms

Graphic Design Request Form

Use this form to request custom graphics for your event or announcement. It helps the design team create what’s needed on time and according to NHM brand standards.

Who should fill out this form:
  • Anyone with an approved and scheduled event that is missing a graphic for promotion only.
  • Anyone whose event has already been approved by admin and is listed on the event calendar, but needs design support to move forward with promotions.
  • This Graphic Design Request Form is only for approved and scheduled events that are not starting from scratch  but simply need a graphic to complete the process.

Who should NOT fill out this form:
  • Do not fill out this form if you are just now submitting a new event for approval.

    Once your event is approved, you will receive a second follow-up email with access to the Event Creation Request Form:

    That form is the full planning request and includes graphic design needs for new events. It only becomes available after your event is approved by admin.
